Normal Saiyan and Oozuru
A normal Saiyan is characterized by black or very dark brown hair, black eyes and a tail. The Saiyans are native to the planet Vegita, which was destroyed, along with the most of Saiyan race, by a changeling named Freeza.
Oozuru
This is the first transformation that was seen by a Saiyan in the original Dragonball. When a saiyan with a tail is looks at a full moon, they transform into a giant, brown-furred, ape-like creature.
This stage multiplies the saiya-jin's power level by 10.
This transformation can only occur when a saiya-jin has a tail, the only way to reverse the transformation is to cut off the tail or remove the moonlight.

All Saiyans (only ones shown are Son Goku, Vegeta, Son Gohan, Bardock and Bardock's henchmen)

Super Saiyan
This transformation is highly powerful and was first seen during Freeza saga in DBZ. The SSJ transformation is first released when a saiya-jin has intense rage, has a high enough Power Level (approx over 100,000) and has a pure heart (Good or Evil). Also in the manga, it was originally stated that the saiyan would have to have his worst fear be realised to transform to SSJ, this however does not follow for characters other than Son Goku, Vegeta and Son Gohan. As a Super Saiyan, the saiyan's hair will turn golden and will rise in the air. Their eyes will also turn green. Once Super Saiyan has been accessed the first time, the Saiyan can transform without needing the rage. As with all SSJ forms, the Saiyan displays a Golden aura of ki.
Characters that achieved SSJ (and their reasons):
Son Goku (Best friend Kuririn died, realised he couldn't protect his friends)
Vegeta (Intense training, realised he couldn't surpass Goku)
Mirai Gohan, in the Future Trunks timeline (unknown)
Son Gohan, in regular timeline (realised he was too dependant on others and felt he hadn't helped)
Mirai Trunks, future timeline (death of Mirai Gohan)

Super Saiyan 3
This Saiyan form was first achieved during the Perfect Cell saga by Son Gohan. This form is vastly superior to SSJ and USSJ as all aspects of fighting are improved with no speed or manuverability loss.
The SSJ2 form is similar to SSJ but the hair becomes longer and has more depth to it, more strands of hair are seen. Sometimes SSJ2 is characterised by bolts of electricity although not always

Goku (in the afterlife/during fight with Majin Vejita)
Vegeta (during fight with Goku as Majin Vejita)

Ultra Super Saiyan
This transformation occurs when a saiyan focuses all of his energy into raw strength.
The USSJ has longer wider hair, becomes very strong and their muscles grow very large. This form could possibly be stronger than even SSJ2 however the downside is the Saiyan becomes very slow and loses manuverability. This also causes stress on the body and essentially this transformation is useless.
The more intelligent fighters such as Goku and Vegeta realised it was worthless and never used the form.

Vegeta (training in Room of Spirit and Time)
Mirai no Trunks (training in Room of Spirit and Time)
Brolli (through rage when seeing Goku again)
Super Saiyan 2

Super Saiyan 4
Some feel SSJ4 is not a valid transformation because of the lack of golden hair and the little influence Toriyama had on GT.
Others feel it is the true ultimate form of a Saiyan because of the appearance of a tail and ape-like features such as fur.
SSJ4 is achieved while the Saiyan is in Golden Oozaru and the Saiyan controls his anger and restores his calmness.
SSJ4 has reddish fur all over the body except the face and central chest area along with a red tail, the SSJ4 has black hair with a strip of hair hanging over each shoulder.
The SSJ4 also has some baggy (and rather cool looking!) pants.
This form is easily the strongest form out of them all as it is controlled power unlike Golden Oozaru.

Goku (controlled his rage as Golden Oozaru)
Vegeta (controlled his rage as Golden Oozaru, after being shot by "Brute Ray"
Gogeta (fusion strength, fusers had abilty to turn SSJ4)
Note: SSJ4 Gogeta had red hair instead of black and brown fur due to the fusion physics

Golden Oozuru
This form was first shown in GT by Goku when his tail had been restored and he looked at the full Earth from planet Plant.
This form is very much like Oozaru except all the fur is a golden colour and much more powerful than regular Oozaru.
Golden Oozaru is very aggressive and cannot be controlled fully in that form.
